If you or your builder has purchased a kit to assemble your own impact glass block windows on site, this Notice of Approval must be provided to the building department.
If you have selected premade non-impact vinyl-framed glass block windows anywhere in the State of Florida, you can provide this document to your building inspector
If you are installing hurricane-rated, non-impact loose glass block on the exterior of your home or building in Miami Dade County, provide this document to your building inspector
If you are installing hurricane-rated, impact glass block windows in Miami Dade County, provide this document to your building inspector
